Fetal cardiac Z scores by gestational age or femur length, and the Cardiovascular Profile Score

Fetal size

Calculate Z scores from

Schneider 2005 references cover 15–39 weeks; Pasquini 2007 (isthmus and duct) 18–37 weeks. Femur length is often the better size marker when dates are uncertain or growth is abnormal.

Measurements (all in mm)

Valves: annulus in diastole (atrioventricular) or systole (semilunar) as in the source studies; ventricles: end-diastole, 4-chamber view. Isthmus and duct: three-vessel-and-trachea (3VT) view or sagittal arch, immediately proximal to the duct insertion.

Cardiovascular Profile Score

Hydrops
Venous Doppler (umbilical vein and ductus venosus)

Heart area / chest area, 4-chamber view at end-diastole (ellipse traces). >0.20 to 0.35: 2 points; >0.35 to 0.50: 1 point; >0.50 or <0.20: 0 points.

Cardiac function
Arterial Doppler (umbilical artery)

Fetal Z scores

Z = [ ln(measured) - (b0 + b1 x ln(x)) ] / RMSE
x = gestational age [weeks], or femur length

Schneider 2005 sites: measurement in cm, FL in cm (page converts from mm)
Pasquini 2007 sites (isthmus, duct): measurement in mm, FL in mm

Cardiovascular Profile Score (Huhta)

Five categories, 2 points each (10 = normal):
hydrops, venous Doppler (UV, DV), heart size (heart/chest area),
cardiac function, arterial Doppler (umbilical artery)
